Please complete this form to place your reservation for the Geoconference "Mother of All E-Lit Dinners", 7:00 PM, 19 August 2001, at Moti Mahal Restaurant, 1031 W Belmont, Chicago. Or you may simply e-mail us your reservation if you are having trouble with the form. We will confirm your reservation within 48 hours of receiving it.

Please do not post sensitive or critical information. You will not be asked for credit card information, nor for any other information of a similar secure nature. When we call you back to confirm your reservation, we will provide you with simple, secure remittance information.

Terms of reservation - please read carefully:
Upon confirmation, you will be responsible for payment for the designated number of dinners reserved in your name. Please remit before the Dinner to ensure availability of your seat(s). The e-poets network reserves the right to cancel unpaid reservations, and avail the seats to other paying guests.

Reservations will be posted in the order they are confirmed, up to and including the limit of 30 seats total for the dinner, so it is to your advantage to reserve and confirm your seats as early as possible. Reservations may not be accepted after 11:59 PM CDT, Tuesday, 14 August 2001.

Information provided in this form will not be shared, sold, or disclosed to any parties outside the e-poets network without due process. The e-poets network reserves the right to cancel reservations and/or dismiss guests when any such guests exhibit a disruptive nature; paid and attending guests who are dismissed will not be eligible for a refund. E-Lit Dinner guests shall respect the management and etiquette of the host restaurant, or risk being considered disruptive and eligible for dismissal.
